Appointments & Reservations

All appointments must be reserved in advance through our secure booking system. A valid card is required to hold your appointment. By booking, you acknowledge and agree to all practice policies.

Pre-Appointment Forms & Consents

To ensure a seamless clinical experience, all required medical forms and consents must be completed at least 24 hours prior to your appointment. These forms are provided within your appointment confirmation and reminder emails.

Completion in advance allows your provider to review your medical history and prepare safely and appropriately for your treatment.

Cancellation & Late Policy

Your appointment time is reserved exclusively for you.

• Cancellations or reschedules require 48 hours’ notice

• Appointments canceled or rescheduled within 48 hours are subject to a fee equal to 50% of the scheduled service.

• Missed appointments (“no-shows”) may be charged 100% of the service fee

• Clients arriving more than 15 minutes late may need to reschedule to maintain schedule integrity for all patients

This policy ensures fairness for all clients and protects dedicated treatment time.

Deposits & Fees

Certain services may require a non-refundable reservation deposit applied toward your treatment. Deposits secure your appointment time and provider preparation.

A 3% non-cash adjustment is applied to credit card transactions and is displayed at checkout. This fee reflects processing costs and is not applied to cash or debit transactions.

Refund Policy

All services and treatments are final sale.
Due to the medical nature of aesthetic and wellness treatments, results cannot be guaranteed and individual responses vary. We do not offer refunds on:

• Injectables

• IV therapy

• Weight management programs

• Packages or memberships

• Prepaid services

Product purchases may be eligible for exchange only if unopened and returned within 7 days.

Treatment Eligibility

All treatments are performed only after a medical assessment confirms candidacy. We reserve the right to decline or postpone treatment if it is not medically appropriate or safe. Treatment plans are determined solely at the provider’s clinical discretion.

Aesthetic medicine is both science and artistry. Outcomes vary based on anatomy, metabolism, lifestyle, and adherence to aftercare. No specific results can be promised or guaranteed. Individual outcomes vary and additional treatments may be required to achieve desired results.

Medical Disclosure & Consent

Certain treatments require medical clearance and signed informed consent. Clients are responsible for providing accurate medical history and medication information prior to treatment.

Failure to disclose relevant medical information may affect treatment safety and results.

Children & Guests

To maintain a calm clinical environment and ensure safety, we kindly request that only scheduled clients attend appointments unless prior arrangements are approved.

Memberships & Packages

Memberships, packages, and prepaid services are non-transferable and non-refundable. Expiration dates apply where indicated.

Privacy & Confidentiality

Revyve Aesthetics + Wellness maintains strict confidentiality of all client information in accordance with medical privacy standards. Your personal and medical information is never shared without consent except as required by law.

Right to Refuse Service

We reserve the right to refuse service to anyone for reasons including but not limited to safety concerns, medical contraindications, inappropriate conduct, or policy violations.

Policy Agreement

By scheduling or receiving services at Revyve Aesthetics + Wellness, you acknowledge that you have read, understood, and agreed to these policies.
