APPOINTMENT SCHEDULING

Appointments are available by advance reservation through our secure online booking system.

A valid payment method may be required to reserve certain appointments or services.

MEDICAL FORMS & CONSENTS

Medical history forms, consent documents, and any required questionnaires must be completed at least 24 hours prior to your appointment.

This allows your provider adequate time to review your medical history and prepare for your visit.

APPOINTMENT CONFIRMATION

Appointment reminders are sent via email and/or text message approximately 48 hours before your scheduled appointment.

Please confirm your appointment promptly to avoid cancellation.

CANCELATION, RESCHEDULING & LATE ARRIVALS

Your appointment time is reserved exclusively for you.

To provide the highest level of service to all clients, we kindly ask that cancellation and rescheduling requests be made at least 48 hours before your scheduled appointment.

Appointments canceled or rescheduled with less than 48 hours' notice may incur a fee equal to 50% of the scheduled service.

Missed appointments ("No-Shows") may be charged 100% of the scheduled service price.

Clients arriving more than 15 minutes late may need to reschedule if there is insufficient time to safely complete treatment.

DEPOSIT & FEES

Certain services require a non-refundable reservation deposit.

Deposits secure your appointment and are applied toward your treatment at the time of service.

Deposits may be transferred to a future appointment when rescheduling with at least 48 hours' notice.

If an appointment is canceled or rescheduled with less than 48 hours' notice, the deposit will be applied toward any cancellation fee. Any remaining balance may be charged to the payment method on file.

A 3% non-cash adjustment applies to credit card transactions. This fee reflects payment processing costs and does not apply to cash or debit card payments.

REFUND POLICY

All treatments, services, memberships, packages, and prepaid services are final sale.

Because aesthetic and wellness treatments are personalized medical services, refunds cannot be provided once treatment has been performed.

This includes: Injectables, IV Therapy, Vitamin Injections, Medical Weight Management Programs, Memberships, Packages, Prepaid Services

Retail products may be eligible for exchange only if unopened and returned within 7 days.

TREATMENT ELIGIBILITY

Every treatment begins with an individualized medical assessment.

Treatment recommendations are based on your medical history, anatomy, treatment goals, and provider evaluation.

For your safety, Revyve Aesthetics + Wellness reserves the right to postpone, modify, or decline treatment if it is determined that treatment is not medically appropriate.

RESULTS DISCLAIMER

Aesthetic medicine is both an art and a science.

Individual results vary based on anatomy, metabolism, lifestyle, previous treatments, product selection, and adherence to aftercare instructions.

While every effort is made to achieve your desired outcome, no specific results can be guaranteed, and additional treatment may be recommended to achieve or maintain optimal results.

MEDICAL DISCLOSURE &INFORMED CONSENT

Clients are responsible for providing complete and accurate medical information prior to treatment.

Please inform your provider of any:

• Medical conditions

• Medications

• Allergies

• Pregnancy or breastfeeding status

• Recent procedures or treatments

Failure to disclose relevant medical information may affect treatment safety and results.

CHILDREN & GUESTS

To maintain a safe, relaxing clinical environment, we kindly request that only scheduled clients attend appointments unless prior arrangements have been approved.

MEMBERSHIPS & PACKAGES

Memberships, treatment packages, promotional pricing, and prepaid services are non-transferable and non-refundable.

Expiration dates apply where indicated.

PRIVACY & CONFIDENTIALITY

Your privacy is important to us.

Revyve Aesthetics + Wellness maintains strict confidentiality of all personal and medical information in accordance with applicable privacy laws and professional standards.

Your information will never be shared without your authorization unless required by law.

RIGHT TO REFUSE SERVICE

Revyve Aesthetics + Wellness reserves the right to decline or discontinue treatment when medically appropriate or when necessary to maintain the safety, comfort, and integrity of the practice.

Reasons may include, but are not limited to:

• Medical contraindications

• Inappropriate or unsafe behavior

• Failure to comply with practice policies

• Incomplete or inaccurate medical information

POLICY AGREEMENT

By scheduling or receiving services at Revyve Aesthetics + Wellness, you acknowledge that you have read, understood, and agree to these policies.
